What causes dark circles, bags or hollows under the eyes?

Often many people blame those dark circles under the eyes on tiredness and lack of sleep or a ‘big night out’. In fact, mostly these dark circles and bags under the eyes are caused by genetics and often run in the family. In some cases though, it can be attributed to ageing, lack of sleep and lifestyle.

Are there other options besides surgery to treat Tear Duct Troughs?

Few people appreciate that dermal fillers can help in this area and think that surgery is the only solution. Lower eyelid surgery can be very helpful in situations where there is lax skin or a ‘bag’ that consists of herniated fat which has prolapsed forward between the bony orbit under the eye and the eyeball.

Surgery is often used in this situation to help remove excess skin and tighten it and also to remove the fatty bag under the eyes, however many people are not interested in undergoing surgery or simply cannot afford it and prefer more conservative affordable measures such as dermal fillers in the tear duct trough.

How can Dermal Fillers in the Tear Duct Trough help dark rings or bags or hollows under the eyes?

Dermal fillers can gently inflate the area under the eye to either soften the hollowing or lessen the darkness by thickening up the layers between the darker underlying muscle and the thin skin overlying the muscle.

In effect, it replaces the missing subcutaneous fat that has lessened over time with the retraction and thinning of the underlying fat pad. It also can smooth the sharp demarcation between the bulging bag under the eye and the adjoining skin, which softens the junction between the two areas.

How are dark circles, bags under the eyes or hollowing in the Tear Duct Trough treated?

Traditionally dermal fillers are injected into the hollows under the eyes or into the tear duct trough.

It is important not to use too much filler in this area due to the risk of over inflation during the next two to six weeks with the filler attracting water into the area.

Nowadays we know ‘less is more’ and it is important to under correct the area with dermal filler to allow the dermal filler to slowly absorb water over the next two to four weeks. This gradually plumps up the area. A specific filler is recommended for this area which has a lower water absorption tendency than the other fillers in the market.

You can often see an immediate slight improvement after the procedure; however it is a gradual process where water is absorbed into the area. It is difficult to predict how much water will be absorbed into the dermal filler in that area therefore an improvement, not ‘perfection’ can only be expected.

It is very important to have realistic expectations of what can be achieved. Because the water absorption into the dermal filler is a slow process, patients sometimes quickly ‘forget’ how they looked prior to the filler and do not appreciate the difference until they look at their ‘before’ photos.

How long does it last?

Dermal fillers have a tendency to last 6 -12 months however in this area, due to lack of movement, the dermal filler often lasts a lot longer. It is very common for the improvement to last at least 18 months or so and often longer however this is variable between individuals.

What are the risks?

As with any insertion of dermal filler in the skin there is a risk of bruising and swelling.

There is also the risk of overcorrection with increased absorption of water into the dermal filler which can create in itself a swelling and exacerbate the bagginess.

The good news is that in the unlikely event of this occurring, an antidote can be injected into the area to dissolve the product within a matter of days.

It is a very, very rare occurrence to cause visual impairment in this area due to this procedure, however this is naturally a concern for people when they are having injections near their eyes.

Will it be painful?

People often expect this procedure to be very painful due to the proximity to the eyeball which is a sensitive organ, however the skin below the eye is surprisingly much less sensitive and tear duct trough filler is usually a comfortable procedure in experienced hands.

Often numbing cream can be used for approximately half an hour before but is not necessary. The procedure often only takes less than 15 minutes to complete.
